He was predeceased by: his parents, Charles Slayback and Marjorie Slayback (Baker); and his siblings, John Slayback, Gerald "Jerry" Slayback, Walter Slayback (Wally), Lowell "Thorny" Slayback and Charlene Slayback.
He is survived by: his significant other Kathy Slayback (Davis); his grandchildren, Benjamin Robert Schreiber (Ben Ben) and Justus Benjamin Schreiber; and his brother Harold "Kenny" Slayback (Margie) of Aurora. He is also survived by numerous nieces, nephews, great nieces, great nephews, great-great nieces, great-great nephews, great-great-great nieces, and great-great-great nephew.
Visitation will be held on Friday, May 2nd 2025 from 11:00 AM to 2:00 PM at the Washington Park East (10722 E Washington St, Indianapolis, IN 46229). A funeral service will be held on Friday, May 2nd 2025 from 2:00 PM to 3:00 PM at the same location.
