She is survived by: her children, Tammi Lyn Swarthout (Dennis), Bonloe Hammer (Tonja), Jeremy Hook and Jeff Hook; her grandchildren, Blake, Dylan, Grant, Damon, Rachel, Reese, Tiffany, Cora, Avin, Dakota and Rocco; and her siblings, Jay Johnson and Wendy Haywood. She is also survived by her family, her friends.
